As the residents begin their fifth year, Meredith faces the drastic consequences of tampering with Derek’s clinical trial and finds her marriage, career and motherhood in jeapordy while the future of Owen and Cristina's marriage remains uncertain. Alex finds himself a pariah amongst the residents as April tries to step up to the plate as Chief Resident in the wake of a giant sinkhole in the middle of Seattle which tests the doctors of Seattle Grace like never before.
